A 14-year-old girl presented with bilateral vision loss for 3 months. Ultra-widefield swept-source OCT angiography (BM-400K BMizar, TowardPi Medical Technology; 160°) revealed extensive areas of nonperfusion, particularly involving the macular area (A, D). Widefield B-scans (24 mm) of both eyes demonstrated no significant macular structural abnormalities (B, E). However, flow within the retinal vessels of the central inner retina was reduced, corresponding to nonperfusion areas in the central macula (C, F). Beta-thalassemia was confirmed by genetic testing, clinical features, and family history. The patient underwent hematopoietic stem cell transplantation 3 months earlier. Retinal perfusion should be monitored even after transplantation to prevent irreversible vision loss.

 

一名14歲女孩因雙側視力喪失就診三個月。超廣視野掃描源OCT血管攝影(BM-400K BMizar,TowardPi Medical Technology;160°)顯示廣泛非灌注區域,特別是涉及黃斑區(A、D)。雙眼寬視野B掃描(24毫米)均未顯示顯著黃斑結構異常(B、E)。然而,中央內視網膜視網膜血管內的血流減少,對應於中央黃斑的非灌注區域(C、F)。β地中海貧血已透過基因檢測、臨床特徵及家族病史確認。患者於三個月前接受造血幹細胞移植。即使移植後也應監測視網膜灌注,以防止無法逆轉的視力喪失。
